New Member Paperwork
Hi there! Welcome to Designed For Thriving! We are so glad to hear you are interested in working with us! There are a few things we need to complete before your first session and have explained and linked everything you’ll need below. Please make sure to read all the instructions carefully. By the end, you should have submitted 3 different forms and paid one $10 membership fee.
STEP 1: Become a Member
Designed for Thriving is a Private Membership Association. If you haven't already, please click here for more information. Then sign the Membership Agreement and Pay your $10 Membership Fee.

This is a one-time, non-refundable membership fee. Please add the item to your shopping card and proceed to check out.
STEP 2: Read & Sign “Member Bill of Rights”
This Member Bill of Rights document was created by the state of Minnesota for the Complimentary & Alternative practitioners and their clients. It outlines our scope of practice as well as your rights as a member of our Association. This document is not a contract but acknowledges that you have received and understand the information.
STEP 3: Read & Sign “Policies & Practices”
This is a series of nine statements we ask that you acknowledge with your initials and then sign at the end.
